yea milling would definitly be the way to do it. It isn't that hard to mill it. The problem that you would face that would be the most difficult is how to mount it. From what I thinking it is a 4 cylinder manifold so it has some funky twists in it and everything. That is the problem you would have. The cutting is not difficult.
and if you get it done by a machine shop make sure they prove to you with a dial test indicator it is flat say within .001"
