Here are the hot tips.
The windshield has to be SUPER CLEAN.
The windshield can not be hot (i.e. - do this at night).
It has to be held tight against the windshield for a reasonable amount of time. If you do not want to do this by hand, use a broom handle or something like that between the mount pad and the seat back.
Oh yes, the glue can not support the entire weight of the mirror till it has cured, so remove the mounting pad from the mirror before you do any of this.

Get some acitone based cleaner before you stick it back on. Windex or ammonia based cleaners do not do the trick. Clean the window and the metal shindig and make sure that all of the old adhesive is gone before putting it back up there.
If that doesn't work, JB Weld, zip ties and duct tape will look right at home along with the high level of Chevrolet interior acoutriments.

Get yourself some cleaner, take a razor blade to the old spot. Then after that take a white scrub pad after it to remove any excess glue.
Oh yeah, forgot to tell you before you clean off the old spot to use a magic marker and on the outside mark the lines of where it once went.
I have always bought the glue kits from Wally World. Take the glue and apply some to the wind shield and some to the metal backing on the mirror, which hopefully you have removed to make things easier. Then place the metal tab in place and hold that sucker there for about 1 minute maybe 2 depending on if you can hold your arm up that long. After that let it sit for a little while and then attach the mirror to the tab and then presto change-o you mother fucking done and off to drink beer with me again at the Ale House
