The E60 is a reliable car. Of course your going to have your normal problems as you would with any car, just be prepaired to spend some money.

I work on BMW's and Mercedes all the time , an average of 3 or 4 a week , along with every other make and model , I hate people who own European cars and then whine about the cost of maintanence and repairs
Come out of the car? Ive seen numerous timing covers on this exact car put on with the engine in the car.
