Overflow tank keeps overflowing!

Overflow tank keeps overflowing!
My car decided to overheat once agian! It overheats only once in a while normally under heavy driving.My resivoir tanks keeps overflowing, and all the coolant leaks out, why does it overflow so much? I have replaced the following:
all coolant hoses
thermostat
temp sensor
the fan is wired up to a switch and is always on.

this might sound stupid, but replace the radiator cap.

water pump doesnt have to leak the fins could be wearing out and not moving water around the engine good enough which would cause it to over heat.
