installed headers.. problems !
Without cats your car is gonna be running like ass, you need some O2 sims for the rear O2s. Your car is constatly trying to set the proper AFR and that ain't gonna happen with out cats.

Didn't realize you still had cats in your setup, my bad. But I am still on the O2 problem side. Either they are not plugged in or they have failed. Check the wiring to make sure nothing got burned on your headers.

pull the ecm fuse to reset the ecm memory. keep it removed for a few minutes. this will clear all sensor memory the ecm learns. check to make sure there is no exhaust leaks. check and make sure you didnt pinch or burn any wires. give me all the codes that were present. check to make sure you didnt fracture/crack any spark plugs.
if it says the maf has low voltage, the maf probably broke some how. if you know anyone else with a 96+ s series, switch the maf and see if it runs any better.. that way you know without buying anything. It could also randomly be the crank sensor. It will cause problems like that but will usually say on the check engine scan. The computer usually pinpoints it pretty well.
i got a 94 LT1 and i had the same problem just yeaterday and it didnt give me any codes but i talked to a buddy of mine and he said chage the o2's so i did and now it runs like a dream awesome response and acc even in low rpm im almost sure its ur o2's amd it did the same thing backfired like crazy and couldnt move it just ran like crap
the rear o2 sensor codes are only used for catalyst monitoring efficiency not for fuel trim. the low voltage code will cause all those symptoms. a scantool to see the data/voltage is the right way to check it. whaen they realy get bad a good palm slap will make the car stumble. no missfire codes huh? i would still check the spark plugs with a test light to see if one craked, and give the boots a good looking at making sure they are snapped onto the plugs/dist
