Default Code 98 problem

i have a 92 mustang gt and for a while my car has been bucking, stuttering, hesitating all at idle and low rshared_pm for about a month and a half now.
also struggling to stay on at cold start and once its found its spot to idle and i got to drive off it will die EVERYTIME i start to drive unless i keep givin it more gas
i have replaced almost all the electrical parts now(coil, ignition switch, wires, plugs, tps, checked tps setting, cap rotor, tried swapping the ecu, reset timing, just recently changed BAP sensor, changed fuel filter, checked the fuel pressure and checked for vacuum leaks) and for a while it seems to get a little better, or so i think at the time. BUT when it warms up it seems to get a little better and than at WOT its better but not wat it should be.
so for a while iv been running codes with the enigine off and the codes r:
15, 54, 67, and 85
15: is on because i keep unplugging things trying to find the problem and the ecu doesnt have time to relearn fast enough

54: is my ACT sensor which i dont have because the explorer intakes dont have them and the car ran fine before without it

67: is on because i leave my car in 1st when the car is off

85: is on because i dont have the emissions canister anymore

all of those r accounted for and not the problem. so i decided to run a test with the car on and came up with codes 54 and 98
again, 54: is my ACT sensor which i dont have because the explorer intakes dont have them and the car ran fine before without it

98: is saying my right o2 sensor is reading lean while running rich
but iv replaced it about 3 -4 time now and theres no difference whatsoever.
they were used but iv swapped them to the left side as well and no other codes r coming up for the left side.
and whenever i take the 02"s out after running for about 5-10 minutes their covered in black exhaust.

if anyone has any ideas please let me know
hopefully hurst or stangitman will see this thread and have some ideas

thank you for any info

does it seem to be missing? sounds like an injector is dumping or a cylinder isnt firing. have you done a compression check or leak down to verify no mechanical failure?

If you have any kind of exhaust leak between the header and the heads,.. even a minuscule leak the O2 will read lean you will get a bank lean code and the ECM will try to correct it and cause the car to run rich. I went through this with my 4.6 GT. I almost bought a new MAF because someone gave me ill advice over it. Then someone told me to get new header studs and gaskets and tighten down the headers. Problem solved. If your running the cardboard/paper style gasket odds are you have a leak. The OEM metal mesh gasket works the best even with headers.

Not saying for certain that this is your problem. But it is an avenue for you to investigate.
