I saw a tire come off a RX-7 at last year's GRM $2008 Challenge.
But that was because the lug threads stripped.
It was exciting, in a fear for your life way, when the tire came rolling toward those of us who were watching. (Sharron Shields saw it first and pushed me toward safety.)

Luckily the tire was diverted so it hit a truck bumper. It would have done some serious damage if it had hit a car, or spectator.

P.S. The Lotus rim looks like a manufacturing error.
