Default Grinds into 1st and Reverse...

I have recently been having an issue with my car that has become a nuisance to me. Every once in awhile when I am engaging the clutch in first gear or reverse, there will be no gear and just a nasty grind. I usually push in the clutch and try again, after this, and it engages and I'm off like nothing ever happened. This happens at complete random. Sometimes it will do it every other time I try to engage the clutch from a stop, other times it won't be apparent for more than a week. To weed out the otherwise useless suggestions, yes the car is IN GEAR when I try to engage the clutch. I'm running on a ACT 6 puck clutch with a stock SR tranny. The combo has worked well up until now, however I don't think the problem is directly related to the clutch, but I could be wrong. This problem is becoming quite cumbersome, since I never know when it is going to do it, and I'm afraid of even driving the car hard, for the thought of this happening on a launch would be disasterous. Most likely I'm SOL for a new tranny. This is my 2nd SR transmission. Any suggestions or info on what the coulprit could be or anyone who has experienced this fill me in.

Unknown, it's off an SR. It shifts perfect, no grinds in any gear, just the problem I stated above.
