Default misfiring?

i have a 94 accord ex. The car keeps misfiring only if am steady on the gas. If i accelerate or let off the gas it won't misfire. I changed plugs, wires, cap and rotor. Still doing the same. I pulled the MAF and found that the little honey cone filter is dirty, so i cleaned it. still the same!! any ideas or thoughts will be appreciated.

also forgot to mension, when changing the rotor i found that it's KIND OF loose. i had space to rotate it a little bit. Is that normal or should it be tight?
