Parts needed:
Clutch and brake pedal(or you can cut the brake but its ghetto), Tranny+shifter, drive shaft, new master cylinder, new slave cylinder.
Optional parts:
Clutch and flywheel, replacement or resurface.

The reason for new master/slave is they are cheap and usually the first thing to go once a swap is done, and trust me you dont want clutch fluid on your carpet or anywhere near paint.

Labor ONLY your prolly looking around 200-250.
Labor and parts your prolly looking around 475-525.

Call and shop around, check sig for further details.
Turn around for 5 spd conversion should be around 1.5-2 days. Depending on how busy the shop is.
