I don't know, my T-5 lasted a long time with no failures. My guess is he shifted it right into 2nd gear instead of 4th. That would cause something like this.
I highly doubt that .. it seems like it would be almost impossible with mine if you wern't tryin.
Some t5's are pretty stout, others break with stock power.
