How can you tell if a header will actually fit your car if buying it second hand? Can you tell if it is bad just by looking at it?

Take a Straight Edge ruler and lay it flat on the header (where it bolts up to the head). If it lays flat its good, if you see a gap underneath the ruler in the middle (or anywhere) then its no good. Another thing....dents are ok but they must be minor. No dents is best. Simple enough.
