even with indoors and its lit by floruscent lighting, the flicker of the lighting is all over and you can never be 100% spot on for every shot. Im usually in auto unless im trying to get a different affect or auto isnt working. Shooting overcast in sunny days adds a warming affect and a lot of times really makes colors pop more.

I shoot a 18% gray card before a shoot after the lighting is set up them do a custom WB.

Depends... I shoot in Auto, Sunny, Cloudy, Flash. For the most part I use Auto. Sunny pretty self explainatory... same with Cloudy. Flash I use for my strobist work.
I shoot in Raw, so if needed, I can adjust white balance.
