i shoot with:
canon rebel xti
canon 50mm f/1.8
sigma 17-70mm f/2.8-4.5
canon 430ex speedlite

to answer your question, that photo was lit by the 430ex speedlite on camera bounced off the white cieling / white wall behind me...

yeah, i normally dont like photos of people for some reason but that one is great! probably helps that the subject looks great too but i like everything about that pic!

I don't think its working here seems pointless. If you are going for Depth of Field it is too strong and shouldn't be even all around. you also missed a spot on the right tire.
I think a light top/backgound blur with a slight desaturation of the background will keep the views eyes on the main subject.

BTW looks like you did radial blur instead of Gaussian blur.
